A healthy tear film has water, oil and mucin in balance. When the oil layer fails, usually because the glands in the lid margins are blocked, tears evaporate too fast. The surface gets irritated and the eye responds with a flood of reflex tears, which is why so many people with dry eye complain of watering. Treating the dryness is what stops the watering.
Grittiness, as though there is sand in the eye. Burning by the afternoon. Blurring that clears when you blink. Redness. Heavy, tired eyes after a day of screens. Contact lenses that used to be comfortable and are not any more. Paradoxically, watering.
Dry eye has several causes and they need different treatment, so the point of the examination is to work out which one you have.
Lubricants are the start and not the whole answer. Preservative free drops if you need them often. Warm compresses and lid hygiene done properly and consistently, which is what actually clears blocked glands. Anti inflammatory treatment where the surface is inflamed. Punctal occlusion where tear volume is genuinely low. And the boring part that works: fixing the environment.
Blink rate drops sharply when you concentrate on a screen. Put the monitor below eye level so your lids cover more of the eye, look at something far away for twenty seconds every twenty minutes, move air conditioning vents so they are not blowing across your face, and take contact lenses out earlier in the evening than you think you need to.
