Cataract is the most common reason patients come to this clinic, and it is the operation Dr. Rinku Das trained specifically to do. Her fellowship in IOL and microsurgery was completed at JPM Rotary Eye Hospital, Cuttack, in 2010.

What a cataract actually is

The lens inside the eye is clear when you are young. With age it clouds. Light scatters instead of focusing, so colours dull, headlights star and glare, and no change of spectacle power fixes it any more. Surgery removes the clouded lens and puts a clear artificial one in its place. It is the only treatment that works.

When to have it done

Not when a scan says so. When it starts costing you something you care about: reading, driving at night, recognising faces, working. A cataract does not have to be ripe before it can be removed, and waiting until it hardens makes the surgery harder rather than safer.

  • Early: a change of glasses still helps, so review and wait
  • Moderate: glare at night, difficulty reading, dulled colour. This is the usual point to operate
  • Advanced: the lens is dense and vision is poor. Still very treatable, but a longer operation

The operation

Phacoemulsification, done under local anaesthesia as a day case. The clouded lens is broken up and removed through a very small opening, and a folded intraocular lens is placed inside the eye where the natural lens was. You are awake, you feel nothing sharp, and you go home the same day.

Choosing the lens

This is the decision that shapes how you see for the rest of your life, so it gets a proper conversation rather than a price list. A monofocal lens gives excellent distance vision and you read with glasses. Other designs trade some contrast for a wider range of focus. What suits you depends on your eyes, your work and how much you mind wearing glasses. One patient on this site chose a TECNIS Eyhance lens and wrote about it afterwards.

Afterwards

Vision usually clears over the first few days. You will be given drops on a tapering schedule and reviewed the next day, then again over the following weeks. Avoid rubbing the eye, dusty places, swimming and heavy lifting for the period you are told. Most people are surprised how ordinary the recovery is.

Questions people ask about this

Is it painful?

No. The eye is numbed. Most patients describe pressure and bright light rather than pain, and it is over in well under an hour.

How soon will I see clearly?

Often within a day or two, sometimes a little longer if the cataract was dense. Your final spectacle power is settled a few weeks later once the eye has stabilised.

Both eyes at once?

No. One eye is done and allowed to settle before the second is planned.

Can a cataract come back?

The lens that was removed cannot cloud again. The thin membrane behind the implant sometimes thickens months or years later, which is dealt with in a few minutes with a YAG laser.

What will it cost?

It depends almost entirely on which intraocular lens you choose. You will be given the full figure in writing before anything is booked. The consultation to work it out is 800 rupees.
